What is sciatica?

Sciatica is pain that travels along the path of the sciatic nerve — the longest nerve in your body, running from your lower back through your hips, buttocks, and down each leg. It’s not a diagnosis on its own; it’s a symptom of an underlying problem irritating or compressing the nerve.

Most cases of sciatica involve one or more of these mechanisms:

  • A herniated or bulging disc in the lumbar spine pressing on the nerve root.
  • Spinal stenosis — narrowing of the spinal canal — pinching nerves as they exit the spine.
  • Piriformis syndrome, in which a deep buttock muscle irritates the sciatic nerve.
  • Spondylolisthesis, where one vertebra slips forward over another.
  • Degenerative disc disease related to aging or wear-and-tear.

The good news: most sciatica is treatable without surgery, and many patients find significant relief through conservative care when it addresses the underlying mechanical cause.


Signs you may have sciatica

Sciatica looks different from person to person, but common signs include:

  • Pain that radiates from your lower back through one buttock and down the leg — typically on one side, not both.
  • A sharp, electric, or burning quality of pain (different from the dull ache of a strained muscle).
  • Numbness, tingling, or “pins and needles” in the leg, calf, or foot.
  • Weakness in the leg or difficulty lifting the foot.
  • Pain that worsens with sitting, sneezing, coughing, or bending forward.

What causes sciatica?

Sciatica almost always starts somewhere in the lumbar spine. The sciatic nerve is formed from nerve roots that exit between the L4, L5, S1, S2, and S3 vertebrae — and anything that crowds those exits or the nerve itself can produce sciatic pain.

The most common cause we see at our Camarillo clinic is disc-related compression: a disc that has bulged or herniated and is pressing on a nerve root. In older adults, spinal stenosis and degenerative changes are more common. In younger and active patients, muscle imbalances and poor mechanics during lifting, sports, or prolonged sitting frequently play a role.

Using a specialized decompression table, we apply gentle, controlled traction to the lumbar spine — creating negative pressure inside the disc that can help draw herniated material away from the nerve root and improve nutrient flow to the disc.

A typical decompression session lasts about 20–30 minutes. Many patients begin to notice changes within a handful of sessions, though the full benefit usually unfolds over a multi-week treatment plan. It’s painless for most patients and requires no anesthesia, incisions, or recovery time.

Chiropractic care and manual therapy

Targeted spinal adjustments can help restore segmental motion in the lumbar spine, reduce muscle guarding, and support the decompression work. We use techniques selected for your specific presentation — gentle low-force methods are available for patients who prefer them or who have conditions that contraindicate higher-force adjustments.

What to expect at your first visit

  2. Physical and neurologic exam. Range of motion, orthopedic tests, neurologic screen (reflexes, sensation, motor strength), and gait analysis.
  3. Imaging review. If you already have an MRI or X-rays, bring the disc or images. If not, we’ll discuss whether imaging is indicated and arrange it locally.
  4. Findings and plan. We’ll show you what we see, explain what’s likely driving your pain, and walk you through a recommended care plan with timeline and expected milestones.
  5. First treatment (if appropriate that day).

Frequently asked questions

How long does sciatica usually last?

For most people, sciatica improves within 4–6 weeks of conservative care. Some cases resolve faster; others — particularly those involving significant disc herniation or stenosis — take longer. Without addressing the underlying mechanical cause, sciatica frequently returns.

Is sciatica permanent?

The vast majority of sciatica cases are not permanent. With appropriate care, most patients experience meaningful relief. Cases involving severe nerve damage or untreated structural problems can become persistent, which is why earlier evaluation usually leads to better outcomes.

Do I need an MRI before treatment?

Not always. We start with a thorough history and physical exam. If your presentation suggests something that would change our care plan — significant neurologic deficit, severe stenosis, or red-flag symptoms — we’ll recommend imaging. Many patients improve without imaging at all.

Will non-surgical spinal decompression work for me?

Spinal decompression is most effective for sciatica caused by disc herniation, bulging discs, and certain types of stenosis. It’s less helpful for piriformis syndrome or sciatica caused by purely muscular issues. After your exam and any imaging review, we’ll tell you honestly whether you’re likely to benefit.

How many visits will I need?
